To send us your existing form:
Log into Cliniko and click Settings
Under the Patients heading, click on Patient form templates
Select your intake form
In the top right of the screen, click
Send us the resulting

That's it! We'll optimise your form for Peptalkr and have you check it s you have the opportunity to provide us with feedback or changes.
