Test and view your Peptalkr intake form responses inside Cliniko

Wanting to check out testing your Peptalkr intake form as if you're a real patient by seeing how the data comes into your Cliniko? Here's the steps.

1️⃣ Check our your forms


You'll find your forms on the πŸ”— Forms tab of your Peptalkr account.

Each form link contains [variables]β€”these are placeholders that help Peptalkr personalise the form for each patient and ensure their responses are properly recorded in Cliniko.

If you test your form without replacing the placeholder variables, they will not land in Cliniko. So let's do that next, it's easy.

2️⃣ Replace the placeholder variables


  1. Create a dummy patient in Cliniko

We recommend setting up a dummy patient in Cliniko for testing purposes.

Once you’ve created this test patient:

  • Open their profile in Cliniko.

  • Copy the patient ID from the URL at the top of the browser.

2. Update your intake form URL

Next, replace the variables in your form link:

Variable

Replace With

[appid]

The real Cliniko patient ID (e.g. 6847974)

[patientfirstname]

Any real or placeholder first name (e.g. Jane) - all this does is show the patient name in the form when they first open it:

Updated for testing:

https://peptalkr.typeform.com/to/example#a=6847974&name=Jane

3️⃣ Complete the form


Now, copy your edited form URL and open it in a NEW browser tab.

Complete the form from start to finish βœ…

You must open the URL in a new browser tab. If you edit the URL inside your already-loaded form, it won't correctly attach the appointment ID.

4️⃣ Checking Cliniko


Default patient information

Basic details like name, address, DOB, and contact info will automatically populate the relevant fields in Cliniko.

Additional questions (health history etc)

Depending on your Peptalkr form settings, these responses will be saved as either:

  • πŸ—‚ A PDF, found under the Files tab of the patient profile

  • πŸ“ A Treatment Note, found under the Treatment Notes tab

πŸ’‘ Note: If your responses are stored as Treatment Notes, they will be authored by the Cliniko practitioner whose API key you connected in Peptalkr.
