In this article, we will give you all the information so that you know how the Peptalkr forms auto-identification feature works for you and your patient, as well as how the form communicates with Cliniko.

Firstly, we would like to make sure you are using the latest version of Peptalkr forms.

Here's how:

Go to your Forms tab in your Peptalkr account. Here you will want to check to make sure that the form URL ends in 'a=[appid]&name=[patientfirstname]' - or similar. This means you are using the most updated version of Peptalkr forms. If either of your URLs are missing this, please let us know, and we can update your forms.

πŸ“ NOTE: the [appid] and [patientfirstname] in the URL will look different for the patient. These areas will be automatically replaced with the patient appointment ID number and the patient's first name. You may recognise the bracketed words from when you use personalisation in your emails as SMS. It works the same way.


Now that you are using the latest version of Peptalkr forms, we can answer some common questions.

Do the forms auto-identify the patient who is filling it out?

Yes! However, you MUST send the form using Peptalkr transactional (smart) emails or SMS. The reason for this is that when the form is sent with Peptalkr, our system will add in the [appid] and [patientfirstname] automatically, so we know who the patient is and which Cliniko profile to update. This is what it will look like when the patient opens the intake form if their name is Abbey:

Does this mean that they don't need to fill out identifying questions in the form?

That depends on whether you would like to confirm these details with the patient or not. These are the questions we still ask about the patient and why:

Title
We ask this to ensure that the patient is addressed accordingly. This is something you may not already have on file. *This question can be removed at your request if you are satisfied that the existing information in all the Cliniko profiles will be correct, and you are happy to not confirm this.*

Have we got your first name right, [patientfirstname]?

This question is to confirm that we have the correct first name and spelling. Sometimes people might have given a nickname over the phone. This also helps if the name has been misheard by staff and there is a typo in the name. If the patient selects No, the next page will allow them to type in their correct first name and this will then be updated in their Cliniko file.

Last name

This is the same reasoning as above. It gives the patient a chance to give the correct spelling of their last name. *This question can be removed at your request if you are satisfied that the existing information in all the Cliniko profiles will be correct, and you are happy to not confirm this.*

Date of birth

This is the same reasoning as above. It gives the patient a chance to give the correct date of birth. *This question can be removed at your request if you are satisfied that the existing information in all the Cliniko profiles will be correct, and you are happy to not confirm this.*

Sex

This is information we probably don't already have from their profile. This will then be updated into their Cliniko profile. *This question can be removed at your request*

Gender

Same as above *This question can be removed at your request*

Mobile number

Here they will add their mobile number and then be asked on the next page to confirm they wrote it correctly. We may already have this information, but we do this to ensure that they have given the correct number as you are likely sending important SMS notifications to this patient. If the number they enter is different to the one we have in Cliniko, the number will be replaced with this one. *This question can be removed at your request*

Email address

Peptalkr operates by sending email and SMS, sol it is imperative we have the correct email on file. The Cliniko profile may already have this information, but we ask them to supply it again, and confirm they entered it correctly. If this email is different to the one we have in Cliniko, their profile will be updated to this new one. *This question can be removed at your request*

Country

Postcode

Street address

Suburb

State/Territory

This group of questions are asked separately and in this order. The way this is asked means it can be entered correctly into their Cliniko profile.*Any of these questions can be removed at your request*

What is your occupation?

This is another required question and will be added to their Cliniko profile. *This question can be removed at your request*

Emergency Contact

Same as above *This question can be removed at your request*

Medicare Number

Medicare Reference Number

This is asked, but can be removed from your form if it is not necessary. *This question can be removed at your request*


​How did you hear about us?

This question gives the patient options of ways they may have heard about you. We recommend you keep it for reporting purposes.


​Is it OK if we send you promotional SMS from time to time relevant to you?

This is a yes or no question. If the patient selects No, the SMS marketing checkbox will be changed in their Cliniko account. They will still receive important non-marketing SMS even if they select no. *This question can be removed at your request*

This ends the section about themselves that is required. All the following questions are about the patient's visit with you and their health history. This can all be edited and changed to whatever suits you best! You can send us a message with what you want these questions to say. You can even send us a PDF or existing form that you use that you would like us to replicate.

How do I attach the form to a smart transaction email or SMS so that it auto-identifies the patient?

Email - The best way to add the intake form to an email is by adding the form link to a button that will be sent to the patient. Here is a video showing you how to do that:

SMS - When you are writing 'What should the SMS say?' on step 6, you can choose from the form to send from the drop-down box. It will automatically attach the form from your Peptalkr forms for you. You can also directly paste the form URL into the SMS body (recommended if you have more than one intake form with us).
